Assessment & Care Planning

A thorough assessment will be conducted by our social worker or nurse that evaluates: medical history, home safety concerns, psycho-social issues, financial stability, and legal plans. A written report for recommendations of the care plan and goal is provided.

Ongoing Care Management

We offer continuous oversight and management of the client’s care, ensuring our recommendations for care needs are met and the care plan is updated as the client’s needs change. Care management is not just for those living at home; our oversight can be tailored to those in assisted living or other healthcare facilities. We serve as the liaison and advocate for care, coordination, and communication between the client, family, and healthcare providers.

Medical Management

Our care managers accompany clients to all healthcare appointments and arrange the follow-up care. If admitted to a healthcare facility or hospital, we interface with the interdisciplinary team to ensure the appropriate care and post-discharge planning is provided. We also assist clients with medication management. Families and other designated individuals are always kept informed of progress and outcomes.

End of Life Arrangements

We assist clients with understanding and securing advanced health care directives. We also facilitate final plans and end of life discussions with the client, family, and other parties involved.

Certified Care Givers

Service begins with an initial assessment by our social worker or nurse to develop a care plan for the assigned home care aide to follow. AgeWell home care aides are Certified Nurse Aides (CNA), Home Health Aides (HHA), or have completed a 40-hour training and certificate program. AgeWell home care aides are registered with the state and undergo clearance through a Federal criminal background check and finger printing.

Companion Services

An experienced, professional member of our AgeWell  staff is assigned to our clients  providing the following services to your loved one:

• Light housekeeping • Grocery Shopping • Errands • Laundry • Meal Preparation • Medication Reminders • Respite care •relief for family caregivers • Community Engagement • Socialization and Activities.

Personal Care

In addittion to comapnanion services, AgeWell home care aides provides personal care services for our clients who are in need of direct care.

•Hourly, up to 24-hour care • Dementia care • Bathing • Grooming and hygiene • Mobility assistance • Transferring and positioning • Toileting and incontinence • Feeding and special diets

Care Oversight

Our qualified team of care managers will frequently assess for any changes in health and make suggestions for changes in care and/or the plan of care, and ensure that family and significant others are notified of changes on an ongoing basis.

AgeWell Referral and Placement Coordinators know that location is key. Our staff will work hard to find facilities close to your family, friends, and physicians to maintain connections with those in your social support and healthcare networks.

Cost is one of the biggest concerns when it comes to finding and selecting a facility. Not only is our referral and placement service FREE of charge, but we strive to find quality facilities that fit within your budget. Our goal is to make sure you are both physically and financially comfortable.
